Instructions
  • 1. Get Started - From the home page, click "Start a New Family Tree". A privacy disclaimer will pop up. Click "Next" to continue.

  • 2. Name Your Tree - Enter a name for your Family Health History Tree family tree and then click "Submit". This will be the name that displays on your final document. For example, "Johnson-Allen Family".

  • 3. Your Information First - You will then be prompted to enter your own health history information first. This will be the baseline to build the rest of the tree and their relationship to you. To add new family members, click "Submit and Enter a New Family Member". Enter their relationship to you as well as any corresponding health conditions for that family member. You may continue to enter additional family members or go to the "Submit and Go to Family Tree View" to view your family tree with the information.

  • 4. Save Your Tree - From the Family Tree View, click "Save Family Tree" and you will be prompted by your computer to designate a name and a location for your file. Please make note of the location and name of your file for future use. For example, "Johnson-Allen Family Health History Tree".

  • 5. Share Tree with others - You can print your family tree and also share it with others via email. To do so, click the "Download PDF of Tree" link, and make note of where you save the file on your computer. Then you can either open the PDF file to view your Family Health History Tree (a link to the Adobe Acrobat software is given at the time of download), or use your email program to send the file to a family member. The process is quick and easy.


Additional Instructions: To retrieve a saved Family Health History Tree - from the home page, click "Upload an Existing Family Tree". Select your saved Family Health History file from your computer and it will be loaded into the program for viewing and editing.

Email Family Tree - This link provides a quick and easy way to email a copy of your Family Health History Tree to a family member, for them to either view it or to edit it with you.
